About the role
Key responsibilities & impact- Provide supportive services and counseling on healthcare and home care programs and services
- Serve as a member of the interdisciplinary team addressing patients’/clients’/residents’ and families’/caregivers’ social, emotional, and economic concerns
- Provide crisis intervention and assist families in understanding medical situations
- Coordinate healthcare programs among patients/clients/residents, families/caregivers, psychosocial and healthcare teams, and communities
- Respond to suspected abuse, neglect, or violence in accordance with professional policies and state laws
- Develop plans of care and obtain resources from social, health, and human services agencies
- Provide referrals, current information, and education regarding available programs and services
- Participate in special projects, care conferences, transitional rounding, and advocacy
- Apply critical thinking, problem solving, and decision-making
- Establish rapport with patients/clients/residents, families/caregivers, communities, and healthcare teams
- Actively participate with healthcare teams
- Depending on department, provide social services for donors and transplant recipients
Requirements
What you’ll need- Bachelor’s degree in Social Work from an accredited curriculum required
- Healthcare and/or mental health hospital experience preferred
- Must possess a license in good standing in state(s) of practice
- In North Dakota: Licensed Baccalaureate Social Worker (LBSW)
