About the role
Key responsibilities & impact- Provide autonomous primary care for children, adults, and seniors without a primary care provider
- Formulate care plans and make clinical decisions for acute and chronic illness
- Promote health and wellness, disease prevention, and chronic disease self-management
- Provide direct supervision, mentorship, orientation, and practical training to Nurse Practitioner students
- Facilitate student learning, provide feedback, monitor progress, and complete evaluations
- Liaise with Lawrence Bloomberg Faculty of Nursing faculty and staff to support high-quality practicum environments
- Diagnose, treat, and prescribe within the RN-Extended Class scope
- Take medical histories and perform advanced physical and psychosocial assessments
- Apply diagnostic reasoning and develop treatment plans, including education, counselling, screening, referrals, and follow-up
- Order and interpret diagnostic imaging reports within scope
- Refer patients to clinical resources and liaise with hospitals, specialists, and community services as patient advocate
- Arrange physician consultation when care is outside the NP's scope
- Support clinic evaluation and quality improvement activities
- Use EPIC electronic medical records and other Ontario primary-care technology
- Use primary care metrics and client experience/outcome data to develop responsive care interventions
- Maintain compliance with professional legislation, College of Nurses of Ontario standards, evidence-based practice, confidentiality, and health and safety requirements
Requirements
What you’ll need- Master's prepared Nurse Practitioner
- Current registration and good standing in the Extended Class with the College of Nurses of Ontario as a Primary Health Care Nurse Practitioner
- Proof of professional liability insurance
- Expected to obtain and maintain an Adjunct faculty appointment with the University of Toronto
- Strong knowledge of all-ages primary health care, chronic disease prevention, and self-management
- 3 years of recent clinical work experience as a Nurse Practitioner; community experience preferred
- Minimum of 5 years of clinical experience as a Registered Nurse in an acute care setting
- Independent judgment and efficient, effective assessment of situations and problems
- Excellent verbal and written communication skills
- Strong interpersonal, leadership, organizational, and complex problem-solving skills
- Ability to promote professional education and advanced knowledge through research and quality improvement activities
- Initiative and strict confidentiality
- Good attendance and work performance record
- Commitment to the University of Toronto and Scarborough Health Network Code of Conduct and health and safety policies
- Ability to facilitate and sustain a safe, supportive clinical learning environment
- Ethical conduct and appreciation of patient, staff, and community diversity
- Ability to use protective equipment and identify/report hazards
- Current Basic Cardiac Life Support (BCLS) certification required
- Specialty certifications, chronic disease self-management training, diversity/equity/inclusion training, professional organization membership, and additional language ability are assets or preferred
