About the role

Key responsibilities & impact

Provide day-to-day support, optimization, testing, and maintenance of Epic Population Health applications
Support Inpatient Case Management, Healthy Planet, and EpicCare Link
Partner with clinical, operational, and technical teams to analyze workflows
Support system enhancements and evaluate software releases
Resolve application-related issues
Support application integration, system upgrades, testing, and ongoing operational activities
Contribute to Sentara's Population Health growth and strategy
Support improvements in patient outcomes, quality of care, and clinician efficiency

Requirements

What you’ll need

Bachelor's Degree in Nursing, Health Information Management, Healthcare Administration, Information Technology, or a related field (Required (Preferred)
5 years of relevant experience may be accepted in lieu of degree
Epic HP (Healthy Planet), IP Clin Doc (Inpatient Clinical Documentation), or Epic Case Management Certification preferred
3 years of relevant healthcare, clinical informatics, application support, or IT analyst experience with a degree
5 years of relevant experience in lieu of a degree
Experience in clinical case management, care coordination, or population health programs required
Knowledge of inpatient nursing workflows and healthcare operations
Familiarity with Epic Healthy Planet, population health management tools, or healthcare technology applications preferred
Experience supporting software testing, workflow analysis, system upgrades, or application optimization activities preferred
Strong analytical, problem-solving, communication, and customer service skills
Ability to work effectively in a remote, collaborative team environment
