About the role
Key responsibilities & impact- Coordinate and deliver services related to Sentrex patient support programs
- Manage all aspects of program support and act as a primary point of contact for assigned health care providers
- Assist prescribing physicians with patient enrolment, including on-site physician-office visits and form completion
- Perform health care provider and patient registration activities
- Process relevant data received during program service delivery
- Answer telephone and web-based inquiries about program services, diseases, and products
- Provide medical support and education to patients and health care providers
- Prepare for and attend specialized pharmaceutical training sessions
- Provide inbound and outbound telephone support, including adherence support
- Administer case-management protocols, including clinical monitoring, data collection, patient follow-up, compliance, therapy disruption/discontinuation monitoring, and adverse-event reporting
- Work with care navigators and physicians to secure therapy funding and investigate alternative funding
- Attend patient meetings and conferences as needed
- Report adverse events to Health Canada and Med Info and enter information into source and third-party systems
- Maintain confidentiality of client and corporate information
- Liaise with physicians’ offices, pharmacies, infusion service providers, and other parties to coordinate patient and physician services
- Participate in ongoing internal and external continuing education
- Provide proactive patient and health care provider support and advocacy, including enrolment, coverage renewals, patient-journey changes, and reimbursement support
- Follow program protocols to ensure continuity of service delivery
- Travel to physician offices as requested
- Address barriers and optimize patient-care processes
- Perform additional duties assigned by the manager
Requirements
What you’ll need- Must be a Registered Nurse in good standing with the College of Nurses of the respective province
- Bilingualism in English and French is required
- 2+ years of clinical experience
- Must be able to provide proof of liability insurance and a current CPR Certificate
- Experience in oncology, gastroenterology, or rheumatology
- Highly collaborative, with effective and practical communication skills
- Experience in Patient Support Programs is a strong asset
- Proven ability to work in a team environment
- Ability to travel within the assigned geographic region up to 75% of the time to support clinics
- Ability to work Monday to Friday between 8:00 a.m. and 8:00 p.m. EST
