About the role
Key responsibilities & impact- Manage and maintain a maximum caseload of 15 families involved with DCFS
- Coordinate services and care to help youth and families reach their goals
- Collaborate with families to identify individualized needs and develop plans of care
- Complete strength-based assessments across all life domains
- Facilitate and lead Family Team meetings, ensuring youth and family voices are heard and respected
- Develop comprehensive plans of care with the Family Team and assign responsibilities
- Include non-traditional services and evidence-based treatments as appropriate
- Guide the team in adjusting plans for anticipated crisis situations
- Provide consultation and support during plan implementation and monitor progress toward goals
- Advocate for families and least restrictive settings
- Complete CANS assessments at designated intervals
- Maintain administrative records through Google Drive, Apricot, ETO case notes, and on-site case files
- Adhere to Wraparound facilitation fidelity standards, including NCFIE and MiiWrap training
- Conduct required home visits and community-based family meetings
- Engage in ongoing professional development and maintain current knowledge of DCFS policies
- Perform other duties as assigned
Requirements
What you’ll need- Bachelor’s Degree required in Counseling, Psychology, Criminal Justice, Social Work, Education, or related program
- Experience coordinating care and/or working with youth and families on a direct service level
- Basic knowledge of the Illinois child welfare system
- Experience working with diverse populations and in diverse communities
- Knowledge of local resources and service providers preferred
- Knowledge of the DCFS SACWIS data system and Child and Adolescent Needs and Strengths (CANS) Assessment preferred
- Strong collaborative, relationship-building, and communication skills
- Ability to work varying hours, including evenings, to accommodate family needs
- Sensitivity to cultural diversity
- Ability to communicate and cooperate with diverse families, professionals, and community groups
- Assertive follow-up with non-engaged youth, family, and supports
- Attention to detail and thoroughness
- Proficient computer skills in Microsoft Windows and internet proficiency, including Google Drive applications
- Effective verbal and written communication skills
- Bilingual in Spanish preferred
- Ability to exercise discretion and ensure confidentiality
- Valid driver’s license and automobile insurance required for work-related travel
- Ability to transport oneself to and from home visits
- Ability to lift up to 25 lbs. when required
