About the role

Key responsibilities & impact
  • Serve as a liaison between contracts, patients, patient families, HHAs, clinicians, aides, and other departments
  • Ensure effective communication and identify concerns during attendance verifications, introduction calls, and other interactions
  • Support delivery of high-quality care
  • Complete required tasks according to agency guidelines and policies
  • Schedule work assignments based on Home Care Worker training and experience
  • Minimize open cases to ensure timely resolution
  • Document communications accurately in HHA Exchange
  • Communicate issues or changes to patients, family members, contracts, clinicians, aides, and other departments
  • Provide necessary information to healthcare professionals
  • Consult with family members and support personnel to ensure ongoing care and identify client needs
  • Advise HHAs of discharges and changes in service days or hours
  • Complete daily, weekly, and monthly reports
  • Manage overtime and travel time requests
  • Use Smart Map and other tools to locate aides, provide directions, and check availability
  • Complete incident reports
  • Manage mutual and linked cases
  • Open and respond to emails, voicemails, and faxes promptly
  • Adapt to agency or industry changes
  • Discuss concerns and questions with the supervisor
