About the role

Key responsibilities & impact

Screen referrals to Mobile Integrated Health (MIH) for appropriateness
Serve as a patient-facing resource for MIH patient triage and remote patient monitoring
Liaise between inpatient/ED, Care Connect Center Care Coordination, MIH, and SNF at Home programs
Screen patients and facilitate linkage to MIH/SNF at Home service coordination
Assist with placing patients in the appropriate MIH program
Provide nursing support to MIH clinicians and patients
Respond to and triage MIH patient/caregiver calls
Identify patients needing MIH visits and coordinate with the MIH Coordinator
Monitor patient vital signs through remote patient monitoring devices and escalate concerns appropriately
Conduct patient outreach in person and by telephone, including home visits as needed
Participate in weekly MIH team SNF at Home meetings
Reinforce program education with care management colleagues, providers, and PCPs
Collaborate with leadership and training on program changes and updates
Follow patient identification, hand hygiene, PPE, and universal precaution procedures

Requirements

What you’ll need

LPN or RN
Required Massachusetts LPN or RN licensure/registration with the Board of Registration in Nursing
Experience coordinating services and applying criteria to determine clinical eligibility for programs
Experience in nursing triage
Experience in home care, VNA, chronic/complex disease management and/or SNF/LTC preferred
Skills in customer service, communication, technical platforms and mobile applications, interpersonal skills, and clinical triage
Ability and willingness to learn and use new technology and procedures
Ability to navigate Epic Care Link, My Chart, Patient Ping, Arcadia, Tiger Connect, Zoom, Jabber, and Outlook
Smartphone mobile device available for business use supporting Tiger Connect during work hours
Ability to maintain current knowledge/certification licensure and provide evidence of continued education
