About the role
Key responsibilities & impact- Respond to referrals for Developmental Services Case Management eligibility for adults with intellectual disabilities and/or autism
- Meet with individuals, families, case managers and other stakeholders during intake to assess client needs and gather information for eligibility determinations
- Enter intake and eligibility information into the OADS Evergreen database by required deadlines
- Complete outreach and education on the intake and eligibility process in school settings
- Provide state case management for a limited number of cases
- Maintain accurate referral reports and submit them to the supervisor in a timely manner
- Gather and document all pertinent facts for eligibility determinations
- Meet with department transition specialists to review individual cases
- Determine when to notify and consult with the Casework Supervisor and others
- Assess crisis situations and emergency intervention needs, notifying supervisory staff when necessary
- Present the role of intake and eligibility to the public in group settings, transition fairs and meetings
Requirements
What you’ll need- Bachelor’s Degree from an accredited educational institution in Social Work/Social Welfare; OR a Bachelor’s Degree in a related social service/social welfare/social work area including at least 12 courses in behavioral science, social science, or social work
- Must have or be eligible for conditional or full licensure as a Licensed Social Worker (LSW) through the Maine State Board of Social Worker Licensure
- At least 1 year of experience working with incapacitated and dependent adults preferred in the background of well-qualified candidates
- Submit a copy of transcripts with application materials
- Possession of a valid Class C Maine Motor Vehicle Operator's License
- Currently, this position is not eligible for visa sponsorship or STEM OPT extensions
- Ability to travel throughout Sagadahoc, Lincoln, Knox and Waldo counties
