About the role
Key responsibilities & impact- Undertake Care Act Assessments
- Develop support plans
- Conduct reviews
- Participate in covering the Mental Health Duty Team
- Work across the North Norfolk area and Mental Health Duty Team
- Work collaboratively with Norfolk and Suffolk Foundation Trust and various stakeholders
- Promote independence
- Engage with people in public spaces and utilise community assets
- Support people to be independent, resilient and well
- Contribute to continuous learning, shared insight and innovation
- Work from home and the Hellesdon Hospital office base, with opportunities to work from other Norfolk County Council bases
Requirements
What you’ll need- Knowledge and experience of the Care Act (2014)
- Knowledge and experience of the Mental Capacity Act (2005)
- Knowledge and experience of the Mental Health Act (1983)
- Knowledge of safeguarding vulnerable adults
- Excellent time management skills
- IT skills
- Ability to work a minimum of two days per week in a community or office setting
- Ability to work Monday to Friday
- Willingness to develop knowledge of Norfolk County Council processes and procedures
