About the role
Key responsibilities & impact- Assess and support adults aged 18+ living in the community across Central Norfolk
- Work with the Physical Disability and Older People’s Team
- Apply the Care Act (2014) to provide proportionate, outcome-focused support
- Support individuals to achieve outcomes that matter most to them
- Use person-centred and strengths-based approaches
- Work collaboratively with Social Workers, Assistant Practitioners and Occupational Therapists
- Collaborate with Norfolk County Council, NHS and wider community partners
- Promote independence, resilience and wellbeing
- Manage complex cases
- Contribute to community-led and personalised solutions
- Reflect on practice, share insights and adapt approaches based on what works
- Contribute to continuous learning, improvement and innovation
- Attend face-to-face interviews at County Hall, Norwich
Requirements
What you’ll need- Qualified Level 2 Social Worker
- Registered with Social Work England as a practising Social Worker
- Experience working with adults and managing complex cases
- Sound understanding of the Care Act (2014)
- Sound understanding of the Mental Capacity Act (2005)
- Understanding of safeguarding responsibilities
- Strong time-management skills
- Strong IT skills
- Full, clean driving licence
- Access to a car
- Commitment to strengths-based, person-centred practice
- Partnership-working approach
- Supporting statement demonstrating suitability against the person specification and job description
