About the role

Key responsibilities & impact
  • Deliver pharmaceutical care through patient-centered clinical services
  • Provide medication information and education
  • Prepare and distribute medications safely, effectively, and cost-efficiently
  • Assess patients and contribute to and implement plans of care based on diagnosis
  • Monitor patients and provide attentive care
  • Communicate with patients, families/support groups, and healthcare providers
  • Comply with local, state, and federal regulations, codes, policies, and procedures
  • Orient and work at provider clinics and specialty pharmacies
  • Provide comprehensive medication management, including initial assessments, toxicity evaluations, symptom management, and reassessments
  • Coordinate and direct work to meet specified deadlines
  • Perform accurate medication data entry and use pharmaceutical and electronic health record systems

Requirements

What you’ll need
  • Graduate of an accredited pharmacy school (PharmD)
  • Registered Pharmacist within 120 days
  • Clinical knowledge and therapy management for Chronic Inflammatory disease states, HIV, and Oncology
  • Excellent written and verbal communication skills
  • Strong critical thinking skills
  • Excellent organizational skills and ability to prioritize and follow up
  • Ability to cope with rapid change and work comfortably in an ambiguous environment
  • Ability to troubleshoot problems efficiently and effectively
  • Ability to provide comprehensive medication management, including initial assessments, toxicity evaluations/symptom management, and reassessments
  • Capability to set priorities among competing clinical and operational priorities and coordinate/direct others to meet deadlines
  • Proficient computer experience and accurate data entry skills in Microsoft Word and Excel
  • Knowledge of medical terminology, pharmaceutical names, pharmaceutical calculations, laws, and regulations
  • Knowledge of pharmacy practice settings, including narcotic delivery and procedures
  • Working knowledge of legal requirements and accreditation standards, including The Joint Commission, Title XXII, DHS, DEA, FDA, and USP
  • General knowledge of Microsoft Office Suite, pharmaceutical technology, EHR, and EPIC
  • Preferred: specialty pharmacist experience in a hospital setting
  • Preferred: board certification such as BCACP, BCPS, BCOP, or CSP
  • Preferred: EPIC and Beacon experience
  • Preferred: ASHP Post-Graduate Year 1 General Pharmacy Residency
