About the role

Key responsibilities & impact
  • Serve as the initial point of contact for potential home health patients
  • Manage new home health referrals and ongoing follow-ups
  • Work closely with Home Health nurses
  • Act as a liaison between patients, families, and referral sources
  • Manage centralized intake for home health locations
  • Perform occasional on-site work for training or other purposes if within driving distance to the center

Requirements

What you’ll need
  • Applications accepted only from AZ, OR, AR, ID, TN, UT, NV, and TX
  • Minimum of 5 years of telephonic experience in a healthcare setting
  • 3–5 years of experience using the Epic healthcare system
  • High school diploma or GED
  • 2 years of recent relevant experience
  • Knowledge of medical terminology, anatomy, and CPT/HCPCS/ICD-9 coding
  • Ability to exercise discretion and make independent judgments
  • Knowledge of Microsoft Office programs including Excel and Word, or similar programs
  • Ability to maintain composure during challenging interpersonal interactions
  • Active listening, interpersonal, and telephone communication skills
  • Organizational skills with attention to detail and follow-up
  • Reliable power and internet services for remote work
  • Dedicated home office with a closing door
  • Cannot be a primary caregiver during business hours
  • Must be able to work varied weekdays and rotating weekends
