About the role
Key responsibilities & impact- Work collaboratively with the Physician, licensed Psychotherapist, consulting Psychiatrist, and other care team members to assure the appropriate level of care and coordination for outpatient services
- Report to the Healthy Young Minds Clinical Manager
- Maintain frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources
- Meet with patients and families
- Gather and report patient functioning data and clinical measurements using the patient care registry
- Communicate between collaborative care team members
- Provide direct patient contact to assigned patients and family members
- Provide direct intervention with patients and families, including crisis intervention and de-escalation where required
Requirements
What you’ll need- Bachelor's degree: BA or BS in psychology, social work, or a related field
- 2 years' recent relevant experience
- Oral and written communication skills
- Interpersonal and time management skills
- Ability to work effectively and prioritize tasks in a fast-paced environment with rapidly shifting priorities and competing demands
- Ability to work independently with a minimum of direction
- Intermediate PC skills and word processing skills required
