About the role

Key responsibilities & impact
  • Provide diabetes self-management education/training and/or medical nutrition therapy to individuals, families, and groups
  • Manage a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle intervention goals, and counseling
  • Coordinate patient care with physicians, pharmacists, case managers, social workers, and other healthcare providers
  • Develop individualized medication, therapy, nutrition, and device protocols
  • Empower patients through techniques and behavior modifications to control diabetes, avoid complications, and improve quality of life
  • Gather required hours toward CDCES exam eligibility where applicable
  • Use diabetes monitoring and management equipment and associated software
  • Provide individual and group education on diabetes and other chronic conditions
  • Enter and update electronic medical records in EPIC
  • Make independent judgments and treatment decisions within scope of practice
  • Recommend medication adjustments to physicians/APCs
  • Work autonomously while managing a regular patient panel
  • Maintain ongoing patient care day to day and week to week
  • Collaborate within diabetes clinics/endocrinology and extend care beyond provider visits through follow-up

Requirements

What you’ll need
  • Bachelor's degree in Dietetics, nutrition, or related field
  • RD—Registered Dietitian
  • BLS—Basic Life Support Healthcare Provider
  • For Diabetes Educator II: CDCES—Certified Diabetes Care and Education Specialist; employees hired prior to 05/03/2024 may obtain it within 2 years
  • For Diabetes Educator II: 2 years recent relevant experience
  • Diabetes self-management education/training and/or medical nutrition therapy knowledge
  • Knowledge of diabetes monitoring and management equipment, including blood glucose monitors and insulin injection devices
  • Motivational Interviewing and behavioral counseling skills
  • Group facilitation skills
  • Ability to gather required hours to become eligible for and sit for the CDCES exam within 2 years of hire
  • Strong written and verbal communication skills
  • Knowledge of adult learning theory
  • Microsoft Office Suite and EPIC proficiency
  • Nutritional therapy knowledge and understanding of human anatomy, physiology, pathology, and medical terminology
  • Ability to make treatment decisions within scope of practice and recommend medication adjustments
  • Ability to work autonomously and collaboratively
  • Ability to care for patients from social age to geriatric
  • For onsite employees, BLS certification is required
