About the role
Key responsibilities & impact- Assess, diagnose, plan, and implement medical care for patients in collaboration with a physician
- Assess patients to identify hospice care needs
- Create and modify plans of care
- Make appropriate referrals
- Provide patient and family education
- Monitor patient status and evaluate the effectiveness of established plans of care
- Serve as a resource for medical and nursing staff
- Collaboratively develop and implement Hospice programs in varied settings
- Provide evidence-based physical, emotional, psychosocial, and spiritual/existential care with patients, families, and the interdisciplinary care team
- Promote patients’ quality of life by relieving suffering through illness, death, and family bereavement
- Cover multiple offices in the greater Roseville Yuba City area
Requirements
What you’ll need- Graduate of an accredited school of nursing
- Master's degree in Nursing
- 5 years recent relevant experience
- Direct patient care experience, preferably in a Hospice environment
- Knowledge of clinical disease entities common to hospice and palliative care
- Knowledge of pathophysiology, disease progression, and pharmacological and non-pharmacological treatment modalities for palliative care
- Knowledge of hospice philosophy, the Medicare hospice benefit, Conditions of Participation, Local Coverage Determinations under the Medicare Fiscal Intermediary, and CHAP
- Demonstrated clinical assessment skills
- Interpersonal and communication skills, both oral and written
- Ability to function independently and as a member of an interdisciplinary team
- Ability to exercise sound clinical judgment in planning and providing patient care
- Ability to recognize the special needs of hospice patients and families, friends, and caregivers in the home
- Ability to communicate effectively, maturely, and caringly with patients, family, friends, and caregivers
- Ability to maintain effective and harmonious working relations with internal and external customers
- Proficiency using cellular phones, smartphones or PDAs, laptop computers, and computer technology
- Function within the full scope of licensure
