Offer individualized assistance to patients, families and caregivers to help overcome health care system barriers
Enhance the patient experience and improve clinical outcomes through seamless navigation throughout the continuum of care
Collaborate with physicians and interdisciplinary teams to triage, coordinate, and consistently manage patient care
Educate patients and serve as a point of contact for patients and families
Develop, implement, and monitor navigation activities and sustainability
Facilitate shared decision making and patient engagement through trusting relationships
Address patients' communication and health literacy needs
Help patients integrate their values and preferences into treatment decisions
Promote positive outcomes for patients, providers, and the Sutter health care system

What you’ll need

Graduate of an accredited school of nursing
RN-Registered Nurse of California
BLS-Basic Life Support Healthcare Provider
5 years recent relevant experience
Knowledge of medical terminology required
Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff
Knowledge of third-party reimbursement helpful
Verbal and written communication skills
Microsoft Office software experience required, including Excel and Word
Must manage, store, prepare, receive, unpack, transport, dispose of, or administer hazardous or potentially hazardous drugs in accordance with USP 800 guidelines, where applicable
