About the role

Key responsibilities & impact
  • Provide clinical care and ongoing management through telehealth and remote care technologies

What you’ll need
  • Must be currently living in Texas or New Mexico
  • Experienced Nurse Practitioner
  • Experience in value-based care
  • Experience in primary care
  • Experience in complex care coordination
  • Experience in virtual care
  • Strong clinical judgment
  • Genuine interest in helping build and grow an innovative care model in a start-up environment
  • Ability to manage medically complex Medicaid and Dual Eligible patient populations
