About the role

Key responsibilities & impact

Provide direct patient-centered care and integrated pharmacy services with physicians, nurses, and other healthcare professionals
Attend medical rounds for decentralized shifts and document pharmacist interventions
Assist in developing and implementing facility-specific medication therapy protocols, guidelines, and reference documents
Conduct Medication Use Evaluations and support pharmacotherapy practice changes
Support Staff Pharmacists and perform general Staff Pharmacist responsibilities as needed
Oversee pharmacy technicians, pharmacy students, and pharmacy interns
Integrate pharmaceutical care programs and medication therapy management throughout the institution
Assist department leaders with compliance with TJC, FDA, CDPH, CMS, CABoP, and ISMP standards
Contribute to pharmacy department research for students and residents
Educate and train physicians, nurses, pharmacy technicians, students, and other healthcare professionals on medication use
Maintain professional competence through staff development, independent study, and professional activities
Practice pharmacy in accordance with applicable regulations and pharmacy policies
Establish professional relationships with colleagues, physicians, and other care providers
Support department operations and professional development
Assist Pharmacy Management with formulary optimization and therapeutic substitutions

Requirements

What you’ll need

Valid CA pharmacist’s license, maintained current at all times
PharmD or BS in Pharmacy
Board Certification in Pharmacotherapy (BCPS)
Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
Good written and verbal communication skills
Good command of basic mathematical calculations
Ability to lift/carry up to 25 lbs
Manual dexterity and mobility
Occasional reaching, stooping, bending, kneeling, and crouching
Prolonged standing/walking
Ability to handle emergency or crisis situations
