About the role

Key responsibilities & impact
  • Provide assessment, ongoing treatment, and management of biopsychosocial aspects of health and well-being for identified patients
  • Participate in the development, implementation, monitoring, and evaluation of programs for the TVFHT patient population
  • Collaborate with an inter-professional healthcare provider team
  • Assist with health promotion, disease prevention, and chronic disease management across patients’ lifespans
  • Initially work in St. Thomas, Ontario
  • Monitor and evaluate service delivery for individual clients, communities, and populations using mental health and addictions measurement tools and technology
  • Provide in-person and virtual care

Requirements

What you’ll need
  • Graduate of an accredited University
  • Registered and in good standing with a regulated health professional College
  • Minimum of five years clinical work experience
  • Extensive mental health clinical training, generally acquired through completion of postgraduate studies
  • Knowledge and experience in mental health treatment and management, including evidence-based treatments such as CBT
  • Advanced mental health knowledge and experience working with adults, children, youth, and families
  • Minimum of 3–5 years supervised clinical practice in mental health
  • Understanding of addictions care and navigation
  • Postgraduate studies such as MSW, OT, RN, BScN, CPMHN(C) or equivalent
  • Membership in a professional college
  • Minimum of 3 years’ experience assessing and managing mental health presentations commonly seen in primary care
  • Experience delivering efficient primary care, including ability to share information and teach
  • Experience developing and facilitating groups
  • Knowledge and proficiency in evidence-based social work methods and practices in primary care settings
  • Knowledge of relevant health care legislation and community mental health resources
  • Understanding of social determinants of health
  • Strong leadership, organization, research, evaluation, time management, and interpersonal skills
  • Ability to work independently and collaboratively as a member of a multidisciplinary team
  • Ability to update knowledge and skills through technology-based opportunities, courses, workshops, and conferences
  • Comfort providing in-person and virtual care
  • Experience using an EMR, Microsoft Office, and measurement-based care platforms
  • Open and non-judgmental approach
  • Effective written and verbal interpersonal and communication skills
  • Successful candidate must undergo a criminal background check and medical screening
  • Must have a Social Insurance Number and, in some cases, a valid work permit
