Work with the Case Manager and Program Manager to develop individuals' skills and foster independence

Provide care, supervision, instruction and/or job coaching in person, remotely or in the community
Participate in weekly online videos
Develop activities, curriculum and schedules for in-person or remote programming
Assist the Case Manager with implementation of Individual Service Plans (ISPs) and Individual Habilitation Components (IHCs)
Assist with personal hygiene, toileting, feeding and ambulation needs as needed
Evaluate and document individuals' progress through daily charting and log notes in client electronic files

High school diploma or general education degree (GED)
Three to six months of related experience working with developmentally disabled individuals or other special populations preferred
Must possess valid California Drivers License
Ability to pass background check, physical, TB and drug test
Ability to push and pull up to 50 pounds
