About the role
Key responsibilities & impact- Complete remote order entry, order verification, and drug verification
- Perform transitions-of-care medication reconciliation
- Navigate multiple pharmacy information systems
- Provide drug information and therapeutic guidance to providers
- Assess medication appropriateness, dosing, interactions, and compatibilities
- Support specialized patient populations, including pediatrics, geriatrics, pregnant/nursing patients, HIV patients, and patients with renal/hepatic impairment
- Document interventions and clinical actions accurately and promptly
- Communicate with physicians and nurses
- Ensure compliance with HIPAA, pharmacy laws, and client policies
- Work across VPNs, automation systems, and multiple PIS platforms
- Participate in ongoing clinical, PIS, and policy training
- Meet productivity and quality standards
- Support multiple hospitals across several states through telepharmacy services
Requirements
What you’ll need- Bachelor's in Pharmacy required; PharmD preferred
- At least 1 year of hospital pharmacy experience strongly preferred
- Active and in good standing IL or CA state pharmacist license required
- Willingness to obtain additional state licenses (IA, NE, HI)
- Strong clinical knowledge and comfort working across multiple systems
- Ability to work evenings, overnights, weekends, and holidays
- Active pharmacist licensure and required credentials/system access
- Cable broadband or fiber optic internet service with at least 10 Mbps download/5 Mbps upload
- Knowledge of IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, and related therapies
- Familiarity with VPNs, automation systems, multiple pharmacy information systems, Google Suite/Microsoft Office, and Snagit
- Compliance with HIPAA, pharmacy laws, and client policies
