About the role
Key responsibilities & impact- Provide non-urgent primary care assessments in person or by telemedicine
- Provide direct clinical assessments, consultations and primary care within the RN extended class scope of practice
- Determine clients’ needs for urgent or emergent care
- Provide telemedicine assessments to determine the need for emergency room visits or other care
- Establish caring and therapeutic relationships with patients
- Provide individual and group education on health topics, including medications for cravings and withdrawal
- Collaborate with interprofessional teams on care planning and follow-up for high-risk or complex health problems
- Assist with ethical and difficult situations using communication and conflict management skills
- Collect and analyze data and prepare reports on program indicators
- Monitor telemedicine services and identify issues requiring management attention
- Recommend service improvements to enhance client and staff safety and quality of care
- Support strategic plans, objectives, programs and services in collaboration with the Executive Director and Clinical Director
- Support development and updating of clinical and IPAC policies
- Work with community and hospital partners to strengthen programs and advance services
- Maintain relationships with external networks, professional organizations and service providers
- Support team learning and professional development
- Follow harm reduction, trauma-informed, women-centred and anti-oppressive practices
- Adhere to occupational health and safety requirements and organizational policies
- Promote client service, collaboration, accountability, inclusion, equity and continuous learning
Requirements
What you’ll need- Master’s Degree in Nursing
- Current registration and in good standing with the Colleges of Nurses of Ontario
- Nurse Practitioner Certification from a recognized program
- Membership with the Nurse Practitioner’s Association of Ontario (NPAO)
- Minimum of 2-4 years of clinical experience in a hospital or community health setting, including 2 years caring for clients and families with substance use, mental health and gambling concerns
