About the role
Key responsibilities & impact- Manage patient flow, triage, and direct patient care
- Serve as liaison between front desk staff, nursing staff, physicians, and clinic manager
- Manage patient populations by developing expected outcomes, defining resource utilization, and setting patient goals with providers
- Provide nursing support and coordination to optimize patient outcomes and quality of life through clinical and educational programs or support services
- Support patient self-management of disease
- Coordinate continuity of care following hospital admissions, discharges, and ER visits
- Manage high-risk patients, including those with multiple co-morbidities or high readmission risk, including a registry
- Assess physical, psychosocial, financial, and environmental healthcare needs
- Educate patients and ancillary personnel about healthcare needs, safety issues, benefits, medications, nutrition, wellness, and diagnostic procedures
- Document assessments, treatment plans, and progress using EMR
- Observe, record, and report patient conditions and reactions to drugs and treatments
- Maintain and review patient records, charts, test results, and examination results; follow up with patients
- Respond to and refer treatment-related patient phone calls
- Instruct patients and families regarding medications and treatment
- Maintain patient confidentiality
- Perform professional, clinical, and technical competencies of the assigned unit or department
Requirements
What you’ll need- High School Graduate
- Bachelor Degree Nursing
- 2 or more years of RN experience
- Current active multistate state Registered Nurse (RN) license in good standing in the state in which you live
- Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association within 14 days
- Basic typing and word processing ability
- Excellent analytical, critical thinking and problem-solving skills
- Previous medical office experience preferred
