About the role
Key responsibilities & impact- Manage patient flow, triage, and direct patient care
- Serve as liaison between front desk staff, nursing staff, physicians, and clinic manager
- Manage patient populations by developing expected outcomes, defining resource utilization, and setting patient goals with providers
- Provide nursing support and coordination through clinical, educational, and support programs
- Support patient self-management of disease
- Coordinate continuity of care after hospital admission, discharge, and ER visits
- Manage high-risk patients, including patients with multiple comorbidities or high risk for readmission, using a registry
- Assess patients' physical, psychosocial, financial, and environmental healthcare needs
- Educate patients and ancillary personnel about healthcare needs, safety issues, medications, diagnostic procedures, nutrition, and wellness
- Use EMR and electronic tools to document assessments, treatment plans, and progress
- Observe, record, and report patient conditions and reactions to drugs and treatments
- Maintain and review patient records, charts, and test results; follow up with patients
- Respond to and refer treatment-related phone calls and provide medication and treatment instructions
- Maintain patient confidentiality
- Navigate new cardiology patients by identifying appropriate providers, obtaining history and medical records, preparing charts, and scheduling patients
- Support assigned physicians during clinic sessions and communicate patient information and care plans
Requirements
What you’ll need- High School Graduate
- Bachelor Degree Nursing
- 2 or more years of RN experience
- Current active multistate state Registered Nurse (RN) license in good standing in the state in which you live
- Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association within 14 days
- Basic typing and word processing ability
- Excellent analytical, critical thinking and problem-solving skills
- Previous medical office experience preferred
