About the role

Key responsibilities & impact
  • Support UVM Health initiatives across Vermont and Northern New York designed to reduce financial, transportation, geographic, digital, and other nonclinical barriers to care
  • Organize project activities and facilitate collaboration among UVM Health partner hospitals, clinical and operational teams, and community organizations
  • Maintain project timelines and deliverables
  • Track project progress and outcomes
  • Gather input from patients, community members, partner hospitals, and regional partners to identify recurring access challenges
  • Inform practical, locally responsive improvement efforts
  • Support focus areas including cost of care, financial assistance, medication affordability, transportation, rural and digital access, and health system navigation
  • Support Community Health Needs Assessment and Community Health Improvement Plan priorities across UVM Health’s service area
  • Focus on project coordination and systems improvement rather than ongoing case management or direct-service navigation
  • Report to the Community Health Improvement Manager and work in strategic alignment with the System Director of Health Equity and Community Health

Requirements

What you’ll need
  • Bachelor’s degree in public health, community health, social work, human services, health care administration, public administration, or a related field required
  • An equivalent combination of education and relevant experience may be considered
  • Two to four years of relevant experience in project coordination, community health, public health, human services, health care access, rural health, care navigation, or a related field preferred
  • Experience supporting projects involving multiple departments, community organizations, or other external partners preferred
  • Experience working with communities or populations experiencing barriers to health care access beneficial
  • Experience with community engagement, data tracking, quality improvement, or grant-funded projects preferred
