About the role

Key responsibilities & impact
  • Maintain a caseload of up to 50 members and conduct monthly in-person visits
  • Review and interview potential members to verify Health Homes Program eligibility
  • Complete referrals when necessary
  • Orient and educate members and families about the Care Coordinator role
  • Initiate care plans and provide education about treatments, procedures, medications, and continuing care requirements
  • Collaborate with healthcare professionals, community and social support services, and company employees
  • Document care evidence and member encounters in the EHR system
  • Check in with members and schedule appointments
  • Communicate with providers and care teams about member progress and needs
  • Explain and translate care information for members when necessary
  • Follow up on referrals to ensure continuity of care and member/family needs are met
  • Reassess care plans for effectiveness and desired outcomes
  • Address member questions and process requests
  • Investigate and direct inquiries or complaints to appropriate staff and follow up on resolution
  • Enroll and disenroll members according to established policies
  • Maintain knowledge of community resources, including housing, financial assistance, transportation, and food assistance
  • Follow protocols for pain, safety, and mental health concerns and create safety plans when necessary
  • Recognize and appropriately report child and elder abuse to Child/Adult Protective Services
  • Follow departmental, companywide, safety, and injury-prevention policies
  • Perform other assigned or required duties

Requirements

What you’ll need
  • Fluent in English, written and verbal
  • Bilingual in Spanish, Russian, Arabic, or Vietnamese
  • Ability to communicate clearly in person, by phone, and electronically
  • Adequate hearing and vision, with corrective devices if necessary
  • Ability to identify problems and develop and implement solutions using logic and related information
  • Excellent organizational skills and attention to detail
  • Commitment to patient confidentiality and ethical healthcare standards
  • Ability to lift, carry, push, or pull 20–25 pounds
  • Ability to climb stairs and navigate uneven terrain
  • Ability to bend, reach, and conduct in-person visits in non-traditional environments
  • Ability to remain stationary and move around the office or travel throughout the community
  • Ability to operate a vehicle and travel to meet members
  • Ability to work in homes, shelters, outdoor settings, hospitals, and community organizations
  • Ability to maintain professionalism and safety in diverse environments
  • Ability to work independently and complete assignments within established policies and procedures
  • Frequent use of computers, keyboards, and handheld/mobile devices
  • Ability to type for extended periods
  • Competence with computers, email, virtual platforms, and Microsoft Office-based programs
  • Bachelor’s degree in sociology, social work, public health, physiology, or another related field
  • Minimum 1+ years of experience in healthcare, social work, or care coordination
  • Current and valid driver’s license with a clean driving record
  • Reliable transportation and proof of auto insurance
  • Current BLS certification from the American Heart Association upon start date
  • Distraction-free home workspace with a secure internet connection
  • Health Homes Program, case management, or behavioral health experience preferred
