About the role
Key responsibilities & impact- Provide support to difficult-to-engage patients and patients with cultural or linguistic barriers through field-based contacts and appointment accompaniment
- Assist the Registered Nurse Care Manager, Behavioral Health Care Manager, and Care Coordinator with delegated panel-management activities
- Support ECM team engagement efforts for eligible patients in clinics and communities
- Help patients navigate healthcare systems and community resources
- Conduct phone and in-person follow-ups with eligible and enrolled ECM patients
- Help patients participate in medical and behavioral healthcare and overcome barriers
- Update the multidisciplinary team and providers on patient barriers
- Engage patients using evidence-based approaches such as motivational interviewing
- Promote collaboration between patients and medical/behavioral teams
- Collaborate with ECM team members on patient care plans
- Participate in weekly systematic and ad hoc case reviews
- Consult with the RN Care Manager or Behavioral Health Care Manager before taking clinical action
- Accompany patients to office visits as needed and within health plan guidelines
- Perform other assigned duties and projects
Requirements
What you’ll need- Fluent in English (written and verbal)
- Bilingual in Spanish
- Ability to communicate clearly in-person, by phone, and electronically
- Adequate hearing and vision, with corrective devices if necessary
- Ability to identify problems and develop and implement solutions
- Excellent organizational skills and attention to detail
- Commitment to patient confidentiality and ethical healthcare standards
- Ability to lift, carry, push, or pull up to 20–25 pounds
- Ability to climb stairs and navigate uneven terrain
- Ability to bend, reach, and conduct in-person visits in non-traditional environments
- Ability to remain stationary and move around the office and community
- Ability to operate a vehicle and travel throughout the community
- Ability to work independently and complete assignments within established policies and procedures
- Frequent use of computers, keyboards, and handheld/mobile devices
- Ability to type for extended periods
- Competence with computers, email, virtual platforms, and Microsoft Office-based programs
- Diploma/GED required
- Minimum 1+ years of experience in healthcare, social services, or care coordination
- Current and valid driver's license with a clean driving record
- Reliable transportation and proof of auto insurance
- Current BLS certification from the American Heart Association upon start date
- Distraction-free home workspace with secure internet connection
- eClinicalWorks (eCW) experience preferred
- Care management experience preferred
