About the role
Key responsibilities & impact- Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk
- Provide medical follow-up visits for chronic disease management and ongoing care
- Perform cognitive screening and staging; escalate for further assessment where indicated
- Prescribe and manage GLP-1 therapy for appropriate patients
- Conduct advance care planning conversations
- Document in the EHR to support accurate Medicare billing
Requirements
What you’ll need- Active, unrestricted NP license in New Jersey
- Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
- Active DEA registration
- Experience with Medicare documentation and E/M coding
- Comfort with telehealth and EHR documentation
- Reliable high-speed internet and a private, HIPAA-compliant workspace
- Experience with comprehensive geriatric assessment
- Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
- GLP-1 prescribing experience
- Multi-state licensure
