About the role
Key responsibilities & impact- Provide support to recipients enrolled in the Trillium STAR program
- Educate members and guardians on navigating criminal justice, behavioral, physical, and social services systems
- Explain community-based services and supports
- Work with recipients’ care teams to provide transition linkages for school, employment, housing, community service opportunities, provider services, and treatment supports
- Coordinate with community resources to address reentry barriers including insurance, disability benefits, transportation, food, clothing, and housing
- Establish relationships with local county jails, prisons, Probation and Parole, and Pre-release officers
- Ensure recipients are connected and provide warm handoffs to needed treatment and support
- Meet justice-involved members in correctional facilities and in the community for care management activities
- Educate TCL members about permanent supportive housing, supports, services, and community resources
- Assist with transition planning and ensure effective delivery focused on quality outcomes
- Use person-centered planning to gather information and address individual needs
Requirements
What you’ll need- High School Diploma or GED
- One (1) year lived experience with a behavioral health condition and/or substance use
- North Carolina Peer Support Specialist Certification
- Must have a valid driver’s license
- Must reside in North Carolina and within 75 miles of Trillium’s South Central Region, specifically Onslow County
- Must be able to travel within catchment as required
- Preference given to applicants who have their own history of criminal justice system involvement, incarceration, or MH/SU recovery
- Education, training, and work experience showing a history of learning and applying skills needed to work with individuals from diverse backgrounds
- All candidates must pass a drug test as a condition of employment
- Resume must include employer name, dates of service, average hours worked per week, essential job duties, education details, and licensure/certification information
