About the role

Key responsibilities & impact
  • Lead clinic onboarding through 30-60-90 day implementation milestones and beyond
  • Serve as the primary point of contact for clinic leadership, PCPs, behavioral health care managers, and billing staff
  • Support provider engagement and referral identification
  • Ensure clinic invoices are paid on time
  • Maintain a physical presence with clinics through regular programming of dinners, trainings/CEU, and other provider/admin engagement events
  • Identify clinics trending below performance targets and develop tailored action plans
  • Serve as a subject matter resource on collaborative care workflows, billing procedures, and tracking reimbursements
  • Navigate clinical partners' EMRs to upload and maintain accurate note sharing between behavioral health teams and clinic partners
  • Facilitate ad hoc consultations between PCP teams and Triplemoon's behavioral health team as needed
  • Contribute to clinic success playbook development as the program scales
  • Own relationships with a portfolio of pediatric primary care clinics from onboarding through ongoing optimization
  • Translate metrics into action, build trust with clinical champions, and ensure clinics achieve meaningful patient outcomes

Requirements

What you’ll need
  • 3–5 years of experience in customer success, account management, or inside sales in a healthcare services model
  • Familiarity with behavioral health, collaborative care, or value-based care models strongly preferred
  • Experience working in a primary care setting, particularly pediatric or family clinics
  • Exposure to a variety of clinical EHRs, including Office Practicum, eClinicalWorks, and Athena
  • Exceptional relationship-building skills across clinical and administrative audiences
  • Experience preparing executive-facing reports or QBR presentations
  • Strong project management and time management discipline
  • Past experience with continuous improvement or operational improvement efforts in a clinical setting
  • Tech-savvy/digital native required
  • Experience using digital collaboration tools such as Google apps, Asana, and Notion
  • Data analytics experience with ability to identify trends and translate insights into action
  • Willingness to dive in and belief that no task is too small
  • Bachelor's degree; clinical experience/credentials preferred
  • Ability to travel for on-site clinic visits, approximately 50% of the time
  • Ability to thrive in a fast-paced startup environment and adapt to evolving priorities
  • Past startup experience preferred
