About the role

Key responsibilities & impact
  • Conduct comprehensive psychosocial, mental health, caregiver burden, social determinants of health, and risk assessments
  • Interpret assessment and screening results to identify priority needs, risks, and interventions
  • Conduct serious illness conversations and facilitate advance care planning and documentation
  • Provide clinical recommendations and escalate concerns to physicians, advanced practice providers, and other clinicians
  • Develop, document, and contribute interventions to individualized care plans
  • Participate in interdisciplinary rounds and case conferences
  • Deliver short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers
  • Identify psychosocial barriers to care and recommend evidence-based interventions
  • Document assessments, interventions, and care plans using electronic systems
  • Establish therapeutic relationships with patients and families
  • Collaborate with multidisciplinary teams to improve quality of life, reduce avoidable utilization, and support timely hospice transitions
  • Support care team debriefings and facilitate periodic memorial rounds

Requirements

What you’ll need
  • Master’s in Social Work (MSW) from an accredited program
  • Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the applicable state(s)
  • Clinical experience in palliative care is required, including experience with advance care planning and serious illness conversations
  • Experience working within an interdisciplinary care model
  • Ability to exercise independent clinical judgment and appropriately escalate concerns or respond in urgent or emergent situations
  • Strong verbal and written communication skills
  • Excellent organizational and time-management skills
  • Proficiency in electronic health records (EHRs), clinical documentation systems, and digital tools
  • Ability to work effectively in a fast-paced, evolving environment
  • Commitment to compassionate, patient-centered care aligned with a value-based care model
  • Must reside in EST
