About the role

Key responsibilities & impact
  • Assess patient needs for social services and provide referrals
  • Facilitate patient access to care and coordinate social service delivery
  • Provide medical case management and advocate for community-based social services
  • Explain service delivery and teach patients to navigate aspects of the health care system
  • Coordinate access to financial assistance, health insurance, medication, transportation, foster care, educational resources, housing, and extended/home care
  • Provide education, information, and referrals to specialized intervention programs, including substance abuse detoxification, rehabilitation, and prevention services
  • Follow up on social service referrals and develop alternate plans with medical and mental health providers and community agencies
  • Navigate patients among institutional and community service providers and resources
  • Maintain communication and collaboration with clinical teams
  • Identify uninsured patients or those needing medication coverage; counsel regarding insurance eligibility and enroll patients as necessary
  • Attend team, staff education, and community-based meetings
  • Develop relationships with community-based health resources
  • Adhere to regulatory policies and procedures

Requirements

What you’ll need
  • High School diploma or GED
  • Two years of social work, human service or related professional experience
  • Demonstrated ability in psychosocial assessment
  • Knowledge of community resources
  • Crisis intervention skills
  • Ability to work in a multi-disciplinary team
  • Ability to communicate clearly and concisely in both oral and written form
  • Bachelor’s Degree in Social Work, Human Services or Counseling preferred
  • Bilingual in English and Spanish preferred
  • Equivalent combination of education and experience may be substituted where applicable
