About the role
Key responsibilities & impact- Assess the social needs of families contacting the service and propose action plans
- Process requests for assistance, intervention, and/or information relating to health and social support services
- Prepare applications for compensation through the MDPH (Departmental Office for Disabled Persons)
- Refer individuals to specialized organizations
- Request specific support services to help people remain at home
- Support applications to institutions and care facilities
- Draft appeals
- Work across the Île-de-France region
- Collaborate with head-office teams and the social worker currently in post
Requirements
What you’ll need- State Diploma in Social Work (DEASS) required
- 8–10 years of experience
- Proven experience supporting people living with psychosocial disabilities
- Knowledge of issues related to psychosocial disabilities
- Knowledge of medico-social services and social legislation
- Strong psychosocial assessment skills
- Ability to write clear, well-structured reports, appeals, and administrative applications
- Comfortable providing support by telephone or through digital tools
- Strong collaborative working skills
- Proficiency with standard office and IT tools
- CV and cover letter required
