About the role

Key responsibilities & impact
  • Provide health care services to patients within local nursing facilities
  • Complete appropriate documentation
  • Administer and order diagnostic tests

  • Prescribe and monitor therapies

  • Create electronic medical records progress notes
  • Assist in providing continuity of care
  • Instruct and educate patients regarding compliance with prescribed therapeutic regimens and health maintenance
  • Travel locally between nursing facilities as needed
  • Participate in an on-call group with support from geriatric physicians and other nurse practitioners

Requirements

What you’ll need
  • Preferred experience in Geriatrics
  • New grads must have at least 5 years of nursing experience to be considered
  • Current New York State license as an RN
  • Current New York State license as a Nurse Practitioner
  • National Certification
  • Minimum Master of Science in Nursing (MSN) degree
  • Possession of an NPI (National Provider Identification) number
  • Successful completion of an approved Nurse Practitioner program
  • Three (3) years of professional nursing experience in a clinical setting
  • Current NYS Registered Professional Nurse License
  • ANCC or AANP certification
  • Current NYS Certified Nurse Practitioner (CNP) license and certification with New York State Education Department
  • Current CPR certification required within one (1) week of hire, with maintenance during employment
  • Certified Prescriptive Privilege in NYS
