About the role
Key responsibilities & impact- Navigate and coordinate the care of patients across the continuum of care
- Monitor and manage resources to transition patients to the appropriate level of care cost-effectively
- Ensure quality outcomes and patient satisfaction
- Assess, plan, coordinate, monitor, and evaluate patients’ plans of care in collaboration with the healthcare team
- Monitor the patient care process and collect data to evaluate and improve care
- Collaborate with physicians and other healthcare workers to provide comprehensive care
- Conduct health risk appraisal, health promotion, and clinical outcome studies
- Educate payers, providers, and the public
- Serve as an information source regarding state and national healthcare trends and initiatives
- Enhance care provision and ensure efficient resource utilization
Requirements
What you’ll need- Associate’s in Applied Science in Nursing
- Minimum of two (2) years of experience in acute/tertiary facility
- NYS Registered Nurse
- Bachelor’s of Science in Nursing preferred
- Five (5) years of experience in acute/tertiary facility preferred
