Key responsibilities & impact

Establish Upward Health’s presence in the community and reach potential patients
Educate patients about Upward Health’s offerings
Engage individuals in meaningful conversations
Facilitate patient enrollment into Upward Health programs
Manage a personal caseload, primarily through phone outreach
Conduct in-person visits and other community-based outreach as needed
Ensure patients understand the full range of available services
Report to the Outreach Manager
Collaborate with team members to support patient outreach and engagement goals
Build relationships with potential patients and provide ongoing support
Overcome objections and challenges during outreach
Adjust outreach strategies to patient needs and preferences
Balance caseloads, conduct timely follow-ups, and meet outreach goals
Document patient information accurately and maintain timely system updates
Connect patients with additional community-based services

What you’ll need

Strong verbal communication and persuasive abilities
Excellent interpersonal skills with the ability to build trust and rapport quickly
Strong organizational and multitasking skills to manage a personal caseload efficiently
Self-motivated with the ability to work independently and meet outreach goals
Comfortable with fast-paced environments and adapting outreach methods to various situations
Proficient in using computer systems for documentation, communication, and managing outreach activities
Flexible and adaptable to outreach methods including phone, in-person meetings, and mailings
Fluent in English and Spanish
Familiarity with local resources and community-based services
