About the role
Key responsibilities & impact- Oversee the day-to-day activities of Upward Health's Care Specialists
- Guide a team including medical assistants, certified nursing assistants, community health workers, and peer support specialists
- Supervise and coordinate Care Specialists' work
- Support training efforts and train new staff on protocols
- Ensure care coordination across medical and behavioral providers
- Provide assistance to patients
- Conduct community outreach and represent Upward Health in the community
- Manage caseloads
- Ensure compliance with Upward Health's policies and procedures
- Manage, analyze, and present data to support clinical and operational improvements
- Maintain relationships with local community resources
- Assist the Manager of Care Coordination with leadership duties
Requirements
What you’ll need- Minimum of 5 years in a healthcare-related field supporting patient care, public health, or population health
- At least 2 years of supervisory experience
- Strong organizational, time management, and communication skills
- Proficiency in Microsoft Excel for data analysis and reporting
- Experience or interest in working with underserved populations, particularly in community health
- Valid driver's license and auto liability insurance
- Ability to perform home visits and outreach
- Knowledge of community resources and services
- Ability to effectively supervise and support a multidisciplinary team
- Understanding of diverse populations and cultural differences
- Ability to work in a dynamic, fast-paced environment with evolving protocols and responsibilities
- Excellent written and oral communication skills
- Problem-solving ability in patient care and team coordination
- Ability to manage and mentor a multidisciplinary team
- Patient-centered care orientation
- Ability to build and maintain relationships with local community resources
