About the role
Key responsibilities & impact- Serve as a core member of a pediatric collaborative care team
- Work with a primary care pediatric practice and psychiatric consultant
- Assess pediatric patients’ mental health needs
- Provide brief therapeutic and behavioral interventions
- Coordinate mental health care for patients on an assigned caseload
- Coordinate care with medical providers, consulting psychiatrists, and other mental health providers
- Screen and assess patients for common mental health disorders
- Provide patient education on mental health disorders and treatment options
- Facilitate patient engagement and follow-up care
- Track treatment response using standardized measures in a registry
- Monitor clinical symptoms and treatment side effects or complications in person or via telephone/audio-visual encounters
- Support psychotropic medication management by monitoring adherence, side effects, and effectiveness
- Provide or facilitate in-clinic and outside psychotherapy referrals
- Participate in scheduled caseload consultations with the psychiatric consultant
- Follow up on treatment recommendations with the patient’s medical provider
- Document patient progress and treatment recommendations in the EHR
- Facilitate referrals for clinically indicated community services
- Liaise with patients’ community supports
- Attend required certification trainings
Requirements
What you’ll need- M.S.W. - Master Of Social Work
- Acceptable credentials may include ACLS, Basic Life Support, LCSW, or LMSW, depending on the role
- Experience working with pediatric populations in a clinical setting, including mental health assessments and evidence-based psychotherapeutic interventions
- Ability to screen and assess patients for common mental health disorders
- Ability to provide patient education and facilitate engagement and follow-up care
- Ability to use standardized measures and registries to track treatment response
- Ability to support psychotropic medication management, including adherence, side effects, and effectiveness monitoring
- Ability to provide brief evidence-based psychotherapeutic/behavioral interventions
- Ability to facilitate evidence-based psychotherapy referrals
- Ability to document patient progress and treatment recommendations in an EHR
- Ability to facilitate community-service referrals and liaise with community supports
- Ability to attend required certification trainings
- Preferred: 1 year conducting cognitive or clinical assessments with children and/or adults with psychiatric disorders
- Preferred skills include computer literacy, evidence-based health care, telemedicine, working with children, patient communication, family communication, and multidisciplinary teamwork
- Driver's license is not required
