About the role

Key responsibilities & impact
  • Support children and adolescents from birth to 21 years of age with developmental disabilities and/or behavioral health needs
  • Work with children’s families/caregivers to assess and identify needs
  • Develop individualized plans of care
  • Organize and monitor services and supports
  • Participate as a member of the Children’s Care Management Team alongside Nursing, Psychiatry, Medical, and Family Supports providers

Requirements

What you’ll need
  • BA/BS in human services or closely related field
  • BA/BS in unrelated field plus minimum 1 year experience in relevant children’s services
  • Knowledge of the system of care in Maine preferred
  • Reliable vehicle with proper insurance
  • Valid driver’s license issued by state of residence
  • Strong interpersonal communication skills
  • Proficiency with written, spoken English
  • Ability to utilize various technology systems for record keeping/plan monitoring
