About the role
Key responsibilities & impact- Provide individualized assistance to patients, families, and caregivers to overcome barriers to care
- Use the nursing process to provide education, resources, informed decision-making support, and timely access to screenings, surveillance, and care coordination
- Identify critical patient resources and navigate patients through healthcare services and systems
- Promote patient health and positive outcomes through multidisciplinary collaboration
- Interface with patients and providers in person, by telephone, and electronically
- Perform assessment, diagnosis, outcome identification, planning, implementation, and evaluation using evidence-based practice
- Coordinate and document safe, quality patient care
- Participate in interdisciplinary rounds, case reviews, case management coordination, performance improvement, and shared governance
- Coordinate specialty clinic and multidisciplinary care, identify service gaps, and bridge resources and services
- Improve patient care timeliness and access
- Analyze community needs assessment data with program leaders and administration
- Provide relationship-based, patient-centered care adapted to diverse populations
- Collaborate with population health, navigation, and Wellstar Cancer Network teams
- Complete competency assessments and required education
- Serve as a preceptor or mentor for nurses and other staff or students
- Stay current on advances in cancer care and oncology/navigation practice
- Promote evidence-based practice, nursing research, and performance improvement
- Participate in data collection and address clinical practice questions using evidence and expert resources
- Support efficient use of human and material resources
- Perform other assigned duties and comply with Wellstar policies, standards, and code of conduct
Requirements
What you’ll need- Associate's degree in Nursing or Nursing diploma required; Bachelor's degree in Nursing preferred
- Minimum 5 years of nursing practice required
- Oncology, pulmonary, thoracic, colorectal, or outpatient case management experience preferred
- BLS, American Red Cross BLS, or BLS Instructor certification required upon hire
- Current RN single-state or RN multi-state compact license required upon hire
- Specialty disease or role-specific certification preferred
- Ability to read, write, and speak English
- Basic computer skills and ability to optimize technology for clinical care
- Strong interpersonal, collaborative, and customer service skills
- Ability to work in a fast-paced environment and respond to emergencies decisively, composedly, and respectfully
- Excellent time management, evidence-based nursing, analytical, and critical-thinking skills
- Ability to work with healthcare leadership, providers, patients, and families
