About the role
Key responsibilities & impact- Perform nursing-process-based patient triage using evidence-based practice and protocols
- Conduct initial and follow-up assessments by telephone or secure messaging
- Determine issue severity and prioritize care, including referrals to in-person or virtual visits and urgent or emergency care
- Collect, update, and review patient health histories
- Use the electronic health record to access patient results and relay provider messages
- Provide pre-procedure and pre-surgery instructions and education
- Conduct post-discharge and post-procedure/surgery follow-up
- Educate patients on preventative screenings, vaccinations, and lifestyle interventions
- Support patients with questions about acute or chronic illnesses, medications, and health concerns
- Coordinate and document safe, quality, and timely patient care
- Communicate with providers and healthcare team members through secure messaging, telephone, and email
- Participate in team meetings, onboarding new remote nursing team members, and remote-care workflow improvement
- Help colleagues use Epic tools, phone systems, and other remote-care technologies
- Complete professional competency assessments and required education
- Identify improvements in remote care delivery and suggest innovative solutions
- Participate in workgroups, committees, and shared decision-making groups
- Plan patient care across the care continuum and help enhance protocols and procedures
- Provide remote nursing-team coverage for absences, vacation, and leave
- Perform other duties as assigned and comply with Wellstar policies, standards of work, and code of conduct
Requirements
What you’ll need- Diploma (Nurse) in Nursing required; Bachelor's in Nursing preferred
- RN - Registered Nurse (Single State) required upon hire
- RN - Multi-state Compact required within 1 year
- BLS, American Red Cross BLS, or BLS Instructor certification required upon hire
- Minimum 2 years of experience with direct patient care in any care setting
- 1 year of experience in a physician practice setting in the specific specialty area preferred
- 1 year of experience with virtual care services preferred
- Excellent interpersonal, verbal, and written communication skills
- Strong computer and technical skills
- Excellent time management and organizational skills
- Effective problem-solving and critical-thinking skills
- Ability to work in a fast-paced environment with constantly changing priorities
- Must uphold the Georgia Professional Nurse Practice Act, American Nurses Association Scope of Practice and Code of Ethics Standards, and Wellstar policies and procedures
