About the role

Key responsibilities & impact

Lead the Connect For Life program using Wider Circle's curriculum to build trust and promote health
Lead virtual and in-person events and create a community of connected members
Strategically plan and host events, including venue search, preparation, and member outreach to confirm attendance
Proactively contact members to encourage participation in events and group activities
Conduct one-on-one home visits and facilitate monthly engagement
Meet monthly requirements for member engagement, attendance, and helping members solve care needs
Record member outreach, attendance, and contact attempts in Wider Circle's digital systems for tracking and reporting
Identify community leaders and invite them to become Wider Circle Ambassadors
Connect with prospective members and develop personalized enrollment plans
Liaise between members, health plans, and each other; help with healthcare services, doctor appointments, and closing care gaps
Achieve membership engagement and growth goals using attendance, feedback, and other metrics
Track and report hours worked
Conduct home canvassing and in-home visits when needed

Requirements

What you’ll need

English/Spanish bilingual ability is a plus
Reliable transportation for home visits; willing to travel up to 90 miles on occasion
Able to work well with others, contribute to a positive environment, and provide leadership when needed
Able to take the lead on community engagement initiatives with minimal direction
Comfortable in a fast-paced, changing environment, and eager to learn and grow
3+ years in community outreach, teaching, or facilitation; healthcare experience is a plus
Associate's degree preferred; 5+ years of relevant experience may be considered instead of a degree
Experience creating or supporting a community of 500+ members
Able to prioritize tasks, stay organized, and follow through on commitments
Comfortable using web-based and app-based systems
Able to manage tasks and responsibilities in a remote environment with little supervision
Available to work outside regular business hours when needed
Willingness to complete pre-employment drug testing and background checks
Excellent communication and presentation skills
Outgoing, positive, and able to engage people in person and virtually
Strong networking and leadership skills, with experience guiding peers
