About the role

Key responsibilities & impact
  • Deliver care management services to Support at Home customers
  • Develop and implement continuity of care through care plans, service coordination, and outcome monitoring
  • Communicate primarily by telephone with customers and Community Aged Care stakeholders
  • Coordinate personalised services aligned with customer goals, preferences, and approved care plans
  • Manage customer budgets within funding parameters
  • Monitor changing needs and emerging risks and respond proactively
  • Build relationships with customers, carers, supporters, and health professionals
  • Apply wellness and reablement approaches to encourage independence
  • Ensure care plans reflect cultural, spiritual, and diverse needs
  • Support informed decision-making and positive risk-taking
  • Maintain high-quality documentation and compliance
  • Work in accordance with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards

Requirements

What you’ll need
  • Certificate III in Individual Support (Ageing), Certificate IV in Aged Care, or equivalent qualification
  • At least 12 months of experience in aged care, disability services, or community services
  • Digital confidence and experience with client management systems
  • Strong communication and relationship-building skills
  • Ability to work independently and in a supportive team
  • Time-management skills to stay organised and meet deadlines
  • Passion for person-centred care
  • Proof of COVID-19 vaccination status, or as required by legislation and WMQ policy
  • Relevant probity checks required by legislation and WMQ Limited policy
  • Current driver’s licence and willingness to travel for work
