About the role
Key responsibilities & impact- Provide, coordinate and monitor a variety of Case Management Services to a designated caseload of Mental Health consumers
- Complete comprehensive assessments of consumers’ strengths, current status and goals
- Develop action plans with consumers to obtain service goals
- Provide crisis assistance when immediate attention is warranted
- Provide daily living assistance to help consumers achieve service plan goals and enhance independent living abilities
- Provide linkage, referral, advocacy and resource-mobilization services
- Complete required records, forms and documentation according to agency guidelines
- Meet or exceed agency productivity and timely paperwork standards
- Interact daily with clients who are mentally ill, seriously emotionally disturbed and/or chemically dependent
- Participate in QA&I, Utilization Management and other agency functions
- Adhere to Case Management Guidelines/Standards noted by TennCare Standards
- Perform related responsibilities as required or directed
- Work under the supervision of the Clinical Manager
Requirements
What you’ll need- Must have skills and proficiencies in the principles, concepts and techniques of the Mental Health Profession
- At least a Bachelor’s degree in counseling, psychology, social work, sociology, or a behavioral science field, or must be licensed as an RN
- Individuals with a Bachelor’s degree in an area other than behavioral health must have fifteen (15) college level semester hours of coursework in behavioral health or one (1) year of work experience in behavioral health setting
- Have a valid driver’s license
- Emergency behavior management training within 90 day orientation period and maintained throughout employment
- One year of behavioral health care experience preferred
- Ability to provide services to pediatric, adolescent, adult and/or geriatric mental health consumers
- Ability to work eight hours per day, five days per week
- Subject to on-call duties as designated by the needs of the agency
