About the role
Key responsibilities & impact- Provide, coordinate and monitor a variety of Case Management Services to a designated caseload of Mental Health consumers
- Complete comprehensive assessments of consumers’ strengths, current status and goals
- Develop action plans with consumers to obtain service goals
- Provide crisis assistance when immediate attention is warranted
- Provide daily living assistance to help consumers achieve service plan goals and live independently or in a home with others
- Provide linkage, referral, advocacy and services to help consumers access and mobilize resources
- Complete required records, forms and documentation according to agency guidelines
- Meet or exceed agency standards for productivity and timely paperwork completion
- Interact daily with clients who are mentally ill, seriously emotionally disturbed and/or chemically dependent
- Perform related responsibilities as required or directed
- Participate in QA&I, Utilization Management and other agency functions
- Adhere to Case Management Guidelines/Standards noted by TennCare Standards
- Work under the supervision of the Clinical Manager
Requirements
What you’ll need- Must have skills and proficiencies in the principles, concepts and techniques of the Mental Health Profession
- At least a Bachelor’s degree in counseling, psychology, social work, sociology, or a behavioral science field, or must be licensed as an RN
- Bachelor’s degree in an area other than behavioral health requires fifteen (15) college level semester hours of coursework in behavioral health or one (1) year of work experience in a behavioral health setting
- Valid driver’s license
- Emergency behavior management training within 90 day orientation period and maintained throughout employment
- One year of behavioral health care experience preferred
- Ability to provide services to pediatric, adolescent, adult and/or geriatric Mental Health consumers
- Ability to work eight hours per day, five days per week
- Employee is subject to on call by the Department as designated by the needs of the agency
