About the role

Key responsibilities & impact
  • Provide pharmaceutical care through direct patient care, operational, educational, and administrative activities
  • Lead and advance medication-use safety initiatives at Berkeley Medical Center and Jefferson Medical Center
  • Monitor patient drug therapy through medical rounds and evaluation of medication regimens, medical problems, laboratory data, and procedures
  • Guide providers and patients on best practices and established care guidelines
  • Assist with admission, discharge planning, and medication education
  • Provide drug information and educational programs to healthcare practitioners and pharmacy staff
  • Participate in medication-use evaluations, quality assurance, performance improvement, clinical research, and administrative activities
  • Review medication orders for accuracy and appropriateness
  • Evaluate compliance with medication safety procedures and recommend evidence-based improvements
  • Document medication errors, unsafe conditions, and near misses; analyze risks and prevent recurrence
  • Promote a medication-safety-focused environment
  • Dispense medications and ensure compliance with drug-distribution policies
  • Manage drug-use control, adverse drug reactions, medication errors, acute and chronic disease states, and medical emergencies
  • Communicate findings and recommendations in the clinical information system
  • Maintain required records according to legal and hospital requirements
  • Supervise pharmacy technicians
  • Participate in performance feedback, orientation, education, scholarly activities, staff meetings, and pharmacy projects
  • Complete written assignments, continuing education documentation, annual clinical competency evaluations, and specialty-accreditation education

Requirements

What you’ll need
  • Current pharmacist licensure through the applicable state board: West Virginia, Pennsylvania, Maryland, or Ohio
  • Obtain Basic Life Support certification within 60 days of hire
  • Preferred: Doctor of Pharmacy (Pharm.D) from an ACPE-accredited school of pharmacy
  • Preferred: Completion of a PGY-2 residency in an applicable clinical field; or PGY-1 residency plus board certification; or PGY-1 residency plus three years of relevant pharmacist experience; or three years of relevant pharmacist experience plus board certification
  • Ability to walk and stand for long periods
  • Ability to lift 30 pounds and push 50 pounds
  • Dexterity to finger, handle, and reach
  • Time management skills
  • Verbal and non-verbal communication skills
  • Teamwork skills
