About the role

Key responsibilities & impact
  • Evaluate, diagnose, and treat patients through phone and video telehealth visits
  • Obtain focused histories and review medical records
  • Develop evidence-based treatment plans
  • Order and interpret laboratory and diagnostic testing when clinically appropriate
  • Prescribe medications in accordance with state and federal regulations
  • Consult with colleagues and specialists to optimize patient outcomes
  • Coordinate referrals and escalation to in-person care when clinically indicated
  • Deliver compassionate, efficient care and support an exceptional patient experience
  • Treat common urgent care presentations including upper respiratory infections, asthma and COPD, rashes and skin infections, allergic reactions, eye infections and injuries, headaches, chest pain and cardiac complaints, and musculoskeletal injuries, sprains, and fractures
  • Contribute to an established 24/7 virtual care program and help expand access across the Yale New Haven Health network

Requirements

What you’ll need
  • Previous telehealth experience required
  • Active Connecticut, New York, and Massachusetts licenses required
  • Controlled Substance Registration
  • Federal DEA Registration
  • Board Certification as an Advanced Practitioner
  • ACLS Certification
  • PALS Certification
  • Strong independent clinical decision-making skills
  • Ability to thrive in a fast-paced virtual care environment
  • Candidates must reside within Connecticut, New York, or Massachusetts
  • Strong clinical knowledge and experience managing common urgent care presentations across all age groups
  • Full-time schedule of four 9-hour shifts per week, including morning, daytime, evening, and weekend coverage
