About the role
Key responsibilities & impact- Provide advocacy, culturally appropriate health education, and information about assigned program objectives to clients and communities
- Provide care coordination, case management, and system navigation to assure access to care
- Build individual and community capacity by teaching people how to manage their own health needs
- Conduct individual and community assessments to determine needs and identify risks
- Conduct outreach, including home visits, to provide education, assessment, and social support
- Document services in compliance with the agency’s quality assurance plan
- Report program progress at required intervals according to funding guidelines
- Create educational and program marketing materials, including newsletters, Facebook content, and website content
- Analyze programs and identify participant characteristics
- Organize activities and fundraising events supporting program objectives
- Maintain agency confidentiality and client privacy
- Maintain current knowledge of services provided to clients
- Follow funding source requirements and applicable CARF standards
- Facilitate delivery of the ACCESS Diabetes Prevention Program, provide participant support and services such as blood pressure measurement and diabetic foot care, and maintain curriculum fidelity
- Serve as a cultural bridge among individuals, communities, and health and social service systems
- Travel to community locations for program delivery and/or deliver programs virtually
- Attend training events, meetings, and conferences as needed for funding requirements
- Translate and interpret information for non-English-speaking clients as needed
- Conduct presentations for audiences of varying sizes and demographics
- Assist clients with enrollment in programs, services, and benefits and refer clients to outside services when needed
- Operate standard office equipment and required software applications
- Perform other assigned duties and responsibilities
Requirements
What you’ll need- Full-time position
- Arabic bilingual ability preferred
- Associate degree required in Business, Marketing, Public Health, Social Work, or Human Services, based on assigned functional area
- At least two years of experience in sales, marketing, public health, social work, or human services
- Experience in group facilitation in person and using a video conferencing platform preferred
- Experience in motivational interviewing and lifestyle change health coaching preferred
- Equivalent combination of experience, education, and/or training may be approved by Human Resources
- Diabetes Prevention Lifestyle Coach certification required at hire if employed in the diabetes program, or must be obtained within 3 months of hire
- Knowledge of community resources, social service agencies, crisis intervention providers, service eligibility rules, funding agency requirements, basic health and wellness, fitness principles, healthy living behaviors, and basic EMR functionality
- Effective oral and written communication skills
- Ability to instruct others, operate standard office equipment, and use Microsoft Office and other required software
- Ability to partner across functional areas, prioritize multiple tasks, meet deadlines, and maintain confidentiality
- English speaking and writing required
- Arabic, Spanish, or another native language of the predominant service population preferred
- Local travel may be required
- Normal business hours, with some additional hours as needed
