About the role

Key responsibilities & impact
  • Provide person-centered case management services to individuals with disabilities, mental health conditions, chronic illness, or terminal/life-altering illness
  • Manage temporary, complex, transitional, and turnover-related cases to ensure continuity and quality of care
  • Correct incomplete case work and complete missing or unfinished tasks
  • Advocate for supported individuals and help them achieve personal goals through community resources and empowerment
  • Assist with new county transfers, including initial visits, service setup, support plans, and signature pages
  • Support and shadow new employees and help develop their knowledge and skills
  • Stabilize cases and discharge or reassign them once up to date
  • Collaborate with individuals, families, guardians, employers, case managers, community providers, and other stakeholders
  • Conduct face-to-face meetings at least twice per year and remote meetings as needed to assess wellness, needs, preferences, and abilities
  • Develop, review, evaluate, and adapt individualized support plans and service allocations
  • Monitor services, Medicaid eligibility, waiver status, and benefit continuity
  • Manage budgets and authorize services while preventing duplication
  • Manage an approximately 50-person caseload across multiple counties
  • Attend required meetings and respond to communications within one business day
  • Document services in the electronic health record/database within one business day
  • Report incidents as a mandated reporter in accordance with Accord policy and Minnesota statutes
  • Represent Accord professionally and serve as a positive role model
  • Complete required training and comply with applicable policies and regulations
  • Travel throughout the Twin Cities Metropolitan area to job sites and meetings

Requirements

What you’ll need
  • Bachelor's Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university; OR a bachelor’s degree from an accredited four-year college or university in another field AND one year of experience in the delivery of social services to the target population as a supervised social worker in a public or private social services agency
  • If you hold a Social Work degree, you are expected to be licensed by the Minnesota Board of Social Work, as required
  • Cross-county training to work across multiple counties
  • Proven record of consistently exceeding minimum billing standards
  • Strong written communication skills
  • Ability to complete required documentation and paperwork within one business day
  • Ability to comply with Accord policies, Minnesota rules and regulations, and contracted or funding-party requirements
  • Ability to travel throughout the Twin Cities Metropolitan area; limited overnight travel possible
