About the role
Key responsibilities & impact- Act as a liaison between the agency and the community regarding patient care
- Prioritize compassionate patient care and patient satisfaction
- Ensure smooth transitions from hospital to home
- Visit patients prior to discharge to set reasonable expectations
- Establish trusting relationships with patients and their care teams
- Establish, maintain, and expand relationships with referral sources, including hospital case managers, medical assistants, and doctors
- Keep detailed records of marketing activities and strategic initiatives
- Identify and resolve service issues, emphasizing patient and referral-source satisfaction
- Work in a mixed remote/in-person environment
Requirements
What you’ll need- High school Diploma/GED
- Minimum 2 years healthcare marketing experience (Home Health preferred)
- Understanding and adherence to external accreditation review standards and applicable state, local, and Federal laws and/or regulations
- Maintaining patient confidentiality and abiding by HIPAA laws/regulations
- Understanding and adherence to company policies and procedures
- Ability to work independently is a MUST
- Resume
- Driver's license for Screening and Selection Services (SASS)

- Must not require employment visa sponsorship now or in the future
